NB Unfortunately we have been advised by Cannock Chase CCG that we will only be able to accept patients that reside within the practice catchment area as detailed above.
When joining the practice all new patients need to sign and hand into reception their completed registration forms. You must complete both the GMS1 form and the New Patient Health Check Form for them to click on.

All new patients over the age of 5 years must have an appointment for a health screening check on joining the practice with the Health Care Assistant.
Appointments
An appointment system is in operation at the surgery. You can make pre-bookable appointments in advance. If you require an urgent appointment for that day please call at 8am. The telephones are manned from 8.00am – 6.30pm Monday to Friday.
Disabled Facilities

A Named GP’ for all patients
Every patient in our practice has a ‘Named GP’ who will be accountable for their care. This GP will periodically review your care in which you receive.
Your ‘Named GP’ is who you are registered under. If you are unsure as to whom this is please ask at reception.
Having a ‘Named GP’ is does NOT stop you from seeing the doctor of your choice at the practice.
